What Does Code 11 Mean?
The diagnosis is inconsistent with the provider type. The diagnosis billed is not within the scope of the billing provider's specialty.
Group Code CO (Contractual Obligation): The provider has agreed to accept the payer's determination. The patient is generally not responsible for this amount.
