Denial Code CO-27: Expenses After Coverage
What Does Code 27 Mean?
Expenses incurred after coverage terminated. The date of service is after the patient's coverage end date.
Group Code CO (Contractual Obligation): The provider has agreed to accept the payer's determination. The patient is generally not responsible for this amount.
Why Does This Happen?
Patient's coverage had already terminated when the service was provided.
How to Resolve It
- 1Verify coverage termination date with the payer
- 2Check for COBRA or other continuation coverage
- 3Bill the patient directly
