What Does Code 39 Mean?

Services denied at the time of authorization, or at the time the claim was processed, because the service was already provided or paid by another provider.

Group Code CO (Contractual Obligation): The provider has agreed to accept the payer's determination. The patient is generally not responsible for this amount.

Another provider has already been paid for the same or overlapping service.
