What Does Code 31 Mean?
Patient cannot be identified as our insured. The payer cannot locate the patient in their system with the information provided.
Group Code CO (Contractual Obligation): The provider has agreed to accept the payer's determination. The patient is generally not responsible for this amount.
Why Does This Happen?
Incorrect member ID, name mismatch, or date of birth error on the claim.
How to Resolve It
